Completion of the Certificate of Achievement in Health Care Navigation/Patient Care Coordination prepares students to assist patients in gaining access to resources that contribute to treatment adherence and overall health improvement. Students graduating from this program will have the relevant skills and knowledge necessary to gain employment as entry-level patient care coordinators. Communication, leadership, medical and legal ethics, and health and safety will be emphasized in this program.
Program outcomes:
1. Demonstrate comprehension of the various ethical issues that arise in the context of patient-centered care in healthcare delivery systems.
2. Apply health education strategies to support population health needs.
3. Demonstrate patient advocacy in community health settings.
4. Engage diversity and difference from a perspective of cultural humility in the performance of patient advocacy in support of patients' access to health care resources.
10 to 15 students annually
This program will provide students with the knowledge, skills, abilities, and dispositions required for employment as an entry level Health Care Navigator/Patient Care Coordinator in a healthcare setting, including hospitals, clinics, and medical practices.
This certificate of achievement will require the completion of 26 units in the major area of study.
